A small amount of preparation before your initial consultation will simplify and streamline your visit.
Please bring with you any information from your primary doctor which led to this referral. For example, if any imaging studies (CT scan, MRI, angiogram or ultrasound) were perfomed, please bring the films or disk with you to your appointment. Reports are useful, but your Surgeon will also want to review the images.
We will ask many questions about your medical history. Any details that you can provide about medical problems, prior operations, medications, drug allergies or previous problems with anesthesia will be helpful. Please also bring all of your current medications.
We'll also need to check your insurance cards and identification. Please be familiar with your own plan's copay policies.
